Music Lessons Guidelines
REGISTRATION
Registration is required for participation in any
Ethos, Inc. activity and must be completed before
lessons, ensembles, or classes begin. Completed
registration forms with full payment should be
sent to: 10 N Killingsworth Street, Portland,
Oregon 97217. Checks and money orders should be
made payable to Ethos, Inc. Please do not send
cash. There is a $20 returned check fee.
FINANCIAL AID
To apply for financial assistance, include a
letter from your child's school documenting their
participation in the free or reduced lunch
program. Group Classes: If your
child is eligible for reduced lunch, the fee for
group lessons will be $60 per term. If your child
is eligible for free lunch, the fee will be $30
per term. Private Lessons: If your
child is eligible for reduced lunch, the fee for 8
private lessons will be $104. If your child
is eligible for free lunch, the fee will be $96.
Your request for financial assistance will not be
considered unless the full payment, the completed
registration form, and the school letter is
received by the registration deadline. If your
child is home schooled or in a school that does
not participate in the free lunch program, please
include proof of income in the form of last year's
income tax form and a recent pay stub. Ethos
staff will contact you to determine the cost of
lessons.
PHOTOGRAPHIC AND VIDEO
RELEASE
Ethos, Inc. occasionally uses publicity
photographs and video footage in its effort to
help bring music education opportunities back to
underserved youth. By submitting this registration
form, you irrevocably grant Ethos, Inc. and its
duly appointed agents the absolute right and
permission to copyright and/or publish or use
photographic portraits, pictures or video images
of you or your child, or in which you may be
included in whole or part, or composite or
distorted in character or form in conjunction with
your name or a fictitious name, or reproduction
thereof in color or otherwise, made through any
media, for art, advertising, or any other lawful
purpose whatsoever. You also grant Ethos, Inc. the
same right and permission to use any statements or
testimonials made by you.
MEDICAL
CARE CONSENT, LIABILITY RELEASE & HOLD HARMLESS
RELEASE
By signing this form you hereby authorize all
medical, surgical, diagnostic and hospital
procedures as may be performed or prescribed by a
treating physician for your child if you cannot be
reached in an emergency. You also agree that
neither you nor your child will bring any claims
of any kind against Ethos, Inc., our music
instructors, operators or sponsors as a result of
any injuries, expenses or damages that you or your
child may suffer in connection with your child's
participation in Ethos, Inc.'s music lessons
program, whether such claims are known, unknown or
may arise in the future. You also agree to
indemnify and hold harmless Ethos, Inc., its music
instructors, operators, and sponsors, from any
injuries, damages, and disability resulting from
your child's participation in Ethos, Inc.'s music
lesson programs. You also understand that no one
associated with Ethos, Inc. is authorized to
alter, modify or waive any of the terms of this
agreement in any way.
Student
Responsibilities
The study of
music can be a joyous and rewarding experience. It
offers a unique opportunity to develop one's
self-expression, artistry, and discipline. In
order to make good progress, you will need to do
the following: Attend lessons regularly and be on
time; Practice everyday in a quiet place. Remember
that quality of practice is as important as
quantity; Keep your music and instrument in good
order and bring both to lessons; Talk to your
teacher about your goals and how to achieve them;
Attend live music performances at every
opportunity; Enjoy yourself and remember to take
pride in being a student of music.
Parental Responsibilities
Parents and guardians are solely responsible for
picking up their children from and taking their
children directly to designated Ethos, Inc.
classrooms and instructors. Ethos, Inc. strongly
encourages parents and guardians to participate
with their children by taking an interest in their
musical endeavor. Adult music lessons are also
offered through Ethos, Inc.
Attendance
Students must pay for all lessons in the course,
regardless of attendance. Students must notify
their teacher at least 24 hours in advance of any
absence. Make-up lessons are not required unless
proper notification is given and are at the
discretion of the teacher. Teacher cancellations
will be made up by arrangement between the
students and teacher.
Discontinuation
Ethos, Inc. reserves the right to discontinue a
student at any time for poor attendance,
non-payment of fees, lack of interest, or
disruptive behavior. If the student is
discontinued for non-payment of fees, he or she
may be reinstated when the account balance is paid
in full.
Full or
rescheduled / Canceled Classes
Our classes must meet minimum and maximum
enrollment standards. Classes may fill quickly or
classes may be canceled due to low enrollment. If
you have registered for a full class or a class
that must be canceled, you will be contacted and
entitled to a full refund. All class times listed
in this catalog are subject to change. Students
registering for a class which is rescheduled will
be notified before the first class.

inclement
weather policy
Parents are encouraged to make individual
decisions regarding children's attendance during
inclement weather conditions. Ethos will be
closed for classes if Portland Public Schools and
Offices close because of weather conditions. When
inclement weather is expected on the morning of a
school day, every attempt is made to decide about
school closures and schedule changes the evening
before. For an unexpected or a less definite
weather situation, every attempt is made to make a
decision as early as possible. Ethos' goal is to
make the most appropriate decision for our
students, staff and patrons based on the available
information.
